Photo Identification Card
(Diver License / State issued ID only /
Georgia -issued Voter I.D.Card / Passport /
Alien Registration Card / U.S. issued Photo Military I.D. /
Social Security Card.
A Criminal Background Check
It is the student's responsiblity to provide the results of a criminal background check. A background check can be obtained at any
Sheriff's Department or County Court House
Their is a non- refundable $25 Application Fee

Certified Nursing Assistants and
Patient Care Technician Clinical
Environment at one of
Atlanta's Historic Leading Hospital.
In ordered to order to participated in the
Advance Clinical students will be asset by the
Program Coordinator
To be Eligible for Advance Clinical students must have the following
Have started Hepatitis B
Series Vaccination
Have current Measles, Mumps,
Rubeola (MMR) Vaccinations
Pass Drug Test and
Background Check
With No Convictions
Special agency which you will be given special instruction on
Admission to the Advance Clinical Program*****
