REGISTRATION

There are 5 ways to register. Simply print and fill out the registration form below and you have the choice to register in person, register online, register by mail, register by fax or by phone.

Note: All applications must be accompanied by a $25 Application Fee, $15 Background Check, and a $250 Deposit to reserve your space. This will be applied to your tuition.

The Application will not be completed until Covenant CNA School Registrar's Office receives your Social Security Card, Driver License / State Id and Background Check. If you have a Felony or certain Misdemeanors you are ineligible for our program.

Prospective students can register online 24 hours a day, seven days a week.

Registration by phone is available Monday-Friday, 10am-5pm (EST). If you call after hours or weekends, please leave a message with your name, reason for your call, and a phone number. Your call will be returned within 24 hours by a Covenant CNA School representative.

Register by Fax:
